To simplify your media production and manufacturing experience, IMS has created online forms for submission with your media. The online forms are available using the navigational bar to the left. Forms are also available via fax, mail and by appointment.
The production and manufacturing forms will enable IMS to accurately and efficiently produce and deliver your product on schedule.
Please note: IMS may require one - or more - of the following forms before we can begin production.
The IPR, or Intellectual Property Rights form is necessary on all duplication and replication orders, for DVDs, audio CDs, and CD-ROMs. The IPR Form ensures that the client has the rights to the material Imperial is being asked to manufacture. The IPR form is a necessary safeguard to protect the rights of everyone involved in creating digital media and is standard for all reputable duplication and replication facilities.
The Track Listing form is only for audio CD duplication and replication orders. The form ensures that the master is consistent with the client’s desired order and length of the project.
The Credit Card Authorization form is for all orders that will be paid for with a credit or debit card. If the order is to be paid by cash or check, there is no need to fill out this form.
The Certificate of Resale form is for companies that are going to resell the product purchased from Imperial to other people, such as a retailer, and you have the proper documents, using a resale certificate allows you to avoid paying taxes twice.
The Artwork Approval/Waiver form is to pre-approve your artwork and waive your right to any further corrections and speed up the production process.
The Media Approval/Waiver form is to pre-approve your audio CD, CD-ROM, or DVD media content and waive your right to any further corrections and finalize the production process.
